Small Grant Request for Application

Susan G. Komen® Northwest Ohio is currently offering Small Grants up to $10,000 to support breast health and breast cancer education projects, direct clinical services and patient assistance projects, and conference projects. Komen® Northwest Ohio’s service area includes Monroe County, Michigan and 23 Northwest Ohio counties (Allen, Auglaize, Crawford, Defiance, Erie, Fulton, Hancock, Hardin, Henry, Huron, Lucas, Logan, Mercer, Ottawa, Paulding, Putnam, Sandusky, Seneca, Shelby, Van Wert, Williams, Wood, Wyandot).

Breast Health and Breast Cancer Education Grants:

  • Purpose: To raise awareness of breast health/breast cancer and increase the capacity of effective breast health education/outreach projects to those that are uninsured, underinsured and/or at-risk (i.e. low level of education).
  • Amount: Up to $10,000.

Direct Clinical Services and Patient Assistant Grants (Pilot Projects):

  • Purpose: To provide direct clinical service or patient assistance through awarded organization for the uninsured and/or underinsured population. Direct clinical service reimbursement must be based on the Ohio Reimbursement Rates for the Ohio Department of Health’s Breast and Cervical Cancer Project and established limits by the Affiliate. Please contact the local Affiliate for current reimbursement rates. Patient assistance projects intended to assist breast cancer survivors currently undergoing treatment with costs such as, but not limited to, transportation, medical equipment, nutrition supplements, co-pays, utilities and rent are eligible a at a maximum of $400.00 per person.
  • Amount: Up to $10,000.

 Conference Grants:

  • Purpose: To partner with other organizations to support local scientific or educational meetings, conferences or symposia with an emphasis on those that enhance the interaction between the advocate, healthcare and scientific communities. The content must focus on breast health/breast cancer or to help further the body of knowledge related to research, diagnosis, treatment, education and/or other issues related to breast cancer.
  • Amount: Up to $10,000.
